1) TO ASSIST IN THE RELIEF OF POVERTY AND SICKNESS FOR ALL RACIAL/ETHNIC GROUPS WITH LONG TERM CONDITIONS IN THE COMMUNITY.
2) TO ASSIST AND SUPPORT IN THE TREATMENT AND CARE OF PEOPLE WITH MENTAL AND PHYSICAL LONG TERM CONDITIONS IN THE COMMUNITY.
3) TO FACILITATE IN THE PROVISION OF STRUCTURED SUPPORT, PRACTICAL ADVICE AND ADVANCE THE EDUCATION AND TRAINING ON HEALTH ISSUES FOR PEOPLE WITH LONG TERM CONDITIONS WITHIN THE COMMUNITY.
