BARDSEY, SCARCROFT AND THORNER) AND WHO ARE SOCIALLY ISOLATED, OR LIVING WITH CHRONIC AND OR DEGENERATIVE CONDITIONS, SO THAT THEY CAN RETAIN THEIR INDEPENDENCE AND CONTINUE TO LIVE ACTIVE LIVES WITHIN THEIR COMMUNITY.
