FOR THE PUBLIC BENEFIT IN THE COMMUNITIES OF BAWTRY AND AUSTERFIELD, THE RELIEF OF THOSE IN NEED BY REASON OF YOUTH, AGE, ILL-HEALTH, DISABILITY, FINANCIAL HARDSHIP OR OTHER SOCIAL OR ECONOMIC DISADVANTAGE BY: (A) PROVIDING SERVICES AND SUPPORT TO SERVICES WHICH RELIEVE SUCH NEEDS, INCLUDING APPLYING FOR, AND MANAGING, SOURCES OF FUNDING; (B) IDENTIFYING WHICH SERVICES ALREADY EXIST AND ASSESSING GAPS AND HOW TO PROVIDE FOR THEM; (C) MAKING SURE THAT THESE SERVICES ARE ACCESSIBLE; AND IDENTIFYING HUBS TO CONNECT RESIDENTS TO PROVIDERS AND PROVIDERS TO RESIDENTS; (D) LIAISING WITH CITY OF DONCASTER COUNCIL (CDC) HEALTH AND WELLBEING OFFICERS TO IDENTIFY SERVICES FOR THE RELIEF OF NEED WHICH RESIDENTS MAY ACCESS; (E) USING THESE CONNECTIONS TO SIGNPOST RESIDENTS WHERE APPROPRIATE.
